The short version

The second chapter of the Sharira Sthana of the Sushruta Samhita is, on its surface, a chapter about the soundness of the reproductive tissues. Read it as a chapter about method instead, and something sharper appears. Sushruta grades what he examines on three separate registers — rupa (colour), vedana (the sensation reported to him), and gandha (smell). Three registers, one subject, one short chapter.

Then he ranks. And when he ranks — when he decides what is sadhya, what is difficult, and what is asadhya — two of the three registers fall silent. Colour is never once cited in the ranking. Sensation is never once cited in the ranking. Every single line of the verdict is decided by smell. Colour tells the physician which dosha; only gandha tells him whether anything can be done at all. This article reads Sharira Sthana chapter 2 as what it structurally is: the clearest surviving statement in the Sushruta Samhita that the nose outranks the eye.

Where Chapter 2 Sits in the Sushruta Samhita

The Sushruta Samhita is arranged in six sthanas, or sections. The Sutrasthana lays the groundwork. The Nidana Sthana classifies disorders. The Sharira Sthana — the section on the body — comes third, and it is the shortest and strangest of the six, because it is the section where the text stops being a manual and becomes a philosophy.

Its first chapter, Sarva-bhuta-chinta Shariram, does not describe a single organ. It describes the cosmos: the twenty-five tattvas of Sankhya, the eight categories of prakriti, the purusha as the twenty-fifth principle, and the five mahabhutas with their attributes and their sense organs. This article does not re-tread that ground.

What matters here is the join. Chapter 1 closes with a list of elemental attributes. Chapter 2 opens, immediately, by examining a substance. And the instrument it reaches for first is the one chapter 1 has just finished defining.

That is unusual. In most of the samhitas, cosmology and clinical method sit in different rooms and are never introduced to each other. Here they are adjacent, and the adjacency is doing work.

The three registers of chapter 2

Sushruta examines his subject on exactly three axes, and he is consistent about it across both halves of the chapter:

  • Rupa — colour. Reddish-black, yellowish or bluish, white, blood-tinged.
  • Vedana — the sensation reported by the person, not observed by the physician. Piercing and cutting, burning and sucking, itching.
  • Gandha — smell. Kunapa (cadaverous), puti-puya (of stale suppuration), mutra (of urine), purisha (of stool).

Three registers go in. Watch which one comes out the other end.

Gandha Belongs to Prithvi: What Chapter One Hands to Chapter Two

The closing verses of Sharira Sthana chapter 1 distribute the sensory world across the five mahabhutas. Each element is given its quality, its sense organ, and its share of the three gunas. The text is compressed almost to the point of being a list, and it reads like this:

Akasha (space) carries shabda, sound, and the ear. Vayu carries sparsha, touch, and the skin, along with the movements of the body and lightness. Teja or agni carries rupa, form and colour, and the eyes — and with them heat, digestion, and the sudden generation of energy. Ap (water) carries rasa, taste, and the tongue, along with fluidity, heaviness, coldness and unctuousness. And prithvi, earth, carries gandha — smell — and ghrana, the nose, along with solidity and heaviness.

Then chapter 1 grades the same five on the gunas: akasha abounds in sattva; vayu in rajas; teja in sattva and rajas; ap in sattva and tamas; and prithvi in tamas.

Hold those two lines together, because their combination is the quiet premise of everything that follows. Smell is the quality of the densest, heaviest, most tamasika of the five elements. It is the sense-object of the element furthest from air and fire, the one that does not move, the one associated with embodiment itself — the text's own word for what prithvi contributes is the fact of having a body at all.

So when a physician smells something, he is, in the text's own scheme, reading the most material register available to him. Colour is agni: bright, mobile, informative about activity. Smell is prithvi: slow, dense, and informative about substance. Colour tells you what is happening. Smell tells you what a thing has become.

Chapter 2 begins one verse later.

A note on why this ordering is not an accident

The mahabhutaindriyatanmatra correspondence is one of the most stable structures in the whole classical corpus. It appears in Charaka's Sharira Sthana, in the Ashtanga Hridaya of Vagbhata, and in the Sankhya sources all three drew on. What is distinctive about Sushruta here is not the scheme — it is that he states the scheme and then immediately uses it, in the next chapter, as a working instrument.

The Named Defects, and Why Most Are Named by Gandha

Chapter 2 opens by declaring what it is about: shuddhi — soundness, clarity, the state of being unmixed. Its subject is the two generative substances of the classical scheme, shukra and shonita. That subject has been handled at length on this site already, in our study of garbhotpatti and the formation of the garbha in the Bhavaprakasha, and in the saptadhatu sequence of the seven bodily tissues. This article is not about the subject. It is about the instrument.

Sushruta names the defective states one after another. Set aside what is being examined and simply list what each state is called, and the pattern is immediate:

Four vessels, identical to the eye. The eye has now told you everything it can.

Six named states. Four of the six are named by their smell. The remaining two — granthika and kshina — are named by consistency, which is the other thing a hand or an eye can establish without instruments. Not one of the six is named by its colour, even though the chapter goes on to describe colour in detail a few lines later.

This is worth pausing on, because it is the sort of structural fact that is easy to read past. A classification scheme reveals its priorities in its nomenclature. Whatever a tradition names a thing by is the property that tradition considers most identifying. Sushruta had colour available. He had sensation available. He named four of six categories by smell.

Naming is not description

Charaka's Indriya Sthana opens with a chapter called Varna-svariya — on colour and voice — and names its categories accordingly. Sushruta, given the same freedom in this chapter, named his by smell. Two great texts, two different senses promoted to the naming register, and each choice tells you what that author thought was hardest to fake.

Rupa Names the Dosha, Gandha Names the Verdict

Now the colour material arrives, and it is genuinely precise. Sushruta maps the vitiating dosha onto colour and onto reported sensation together:

This is a tidy, complete, four-row correspondence. It is exactly the kind of table a physician would want. And it answers precisely one question: which dosha is involved.

Cover it and colour is gone. The other evidence is still entirely available.

Then Sushruta reaches the part of the chapter that actually decides something — the verses that sort the states into what can be worked with and what cannot. And the colour table simply does not appear. Not as a qualifier, not as a tiebreaker, not as a footnote. The reddish-black, the yellowish, the white and the blood-tinged are never mentioned again in the ranking.

What decides the ranking is smell, and consistency where smell is not available.

It would have been the easiest thing in the world for the text to say that the vata-coloured states are hardest, or that kapha-white ones respond best. Plenty of later works do exactly that sort of thing. Sushruta declines. Having built a colour scheme, he sets it down at the threshold of the verdict and picks up his nose instead.

Why this division makes sense on the text's own terms

Go back to the elemental assignment. Rupa is agni — and agni in this system is the principle of transformation: heat, digestion, the sudden generation of energy. Colour therefore reports a process, and a process is by definition still running. Gandha is prithvi — solidity, heaviness, embodiment. Smell reports a state, and a state is what a thing has settled into. If you want to know what is happening, ask the eye. If you want to know what something has become, ask the nose. Sushruta's chapter behaves exactly as its own cosmology says it should.

Vedana: The Witness the Physician Cannot See

The third register deserves its own note, because it is the one modern readers tend to skip and it is quietly the most sophisticated thing in the chapter.

Vedana — the piercing, the burning, the itching — is not something Sushruta observes. It is something he is told. It enters the chapter as testimony, not as observation, and the text handles it accordingly: it uses vedana to help identify the dosha, alongside colour, and then it too is set aside when the verdict is made.

The classical framework has a name for this distinction. Charaka's Vimana Sthana, chapter 4, sets out the trividha pariksha — the three means of examination: pratyaksha (what the examiner perceives directly), anumana (what is inferred), and aptopadesha (what is received from authoritative teaching). Chapter 8 of the same section expands the examination of the person into the well-known dashavidha pariksha, the tenfold assessment, which we have examined separately in our article on Vagbhata's dashavidha pariksha in Ashtanga Hridaya Sutrasthana chapter 12.

Within pratyaksha, the classical authors are careful about which senses the examiner may use directly. Four of the five are available to him: he may hear, touch, see and smell. The fifth — rasana, taste — is not exercised directly by the physician on what he examines; where taste matters it is established by inference and by report, famously by observing what ants and flies do. So the examiner's directly usable senses number four, and of those four, smell is the only one assigned to the heaviest element.

Seen that way, Sharira Sthana chapter 2 is not eccentric at all. It is using the deepest register that direct perception makes available, and reserving it for the heaviest decision.

Sadhya, Krichhra-Sadhya, Asadhya: A Ladder Built on Smell

Here is the passage the whole article has been walking towards. After describing the states, Sushruta sorts them into three tiers. The wording of the translation is plain:

Rendered with the Sanskrit prognosis terms restored in place of an older translator's looser English, the passage runs: those that are cadaverously smelling, those clotted and shreddy, those resembling stale suppuration and those thinned are corrigible only with the greatest difficulty — krichhra-sadhya. Those carrying the smell of stool or of urine are asadhya. The remaining states are sadhya.

Notice the internal grading. It is not simply smells bad versus does not. All four of the difficult states carry an unpleasant character, and two of them — kunapa and puti-puya — are frankly foul. They are still placed in the middle tier. Only two specific smells fall into the lowest tier, and they are chosen with what looks like real deliberation.

Which raises the obvious question: what do those two have that kunapa and puti-puya do not?

When a Dhatu Smells of a Mala

The answer is the finest thing in the chapter, and the text does not spell it out — it simply arranges the categories so that a reader who knows the wider system will see it.

The classical body is built from dhatus — the sustaining tissues — and it discards malas, the wastes. Mutra (urine) and purisha (stool) are the two principal malas of the entire system. They are not merely unpleasant substances; they are the substances the body has formally finished with. The relationship between dhatu and mala at every stage of tissue formation is the whole subject of our article on dhatu, mala and ojas in the Bhavaprakasha, and of the sequence traced in dhatu poshana and the kedari-kulya nyaya.

The same clay, the same colour. One of them has stopped being a vessel.

So read the lowest tier again with that in mind. Kunapa is the smell of something dead. Puti-puya is the smell of something stale. Both are bad, and both are still the smell of this substance, gone wrong. But mutra-gandhi and purisha-gandhi are different in kind. They are the smell of a different category altogether. A sustaining tissue that smells of urine or of stool is, in the text's own logic, no longer reporting itself as a dhatu at all — it is reporting itself as a mala.

The distinction the ladder is actually drawing

Middle tier: the substance is degraded. It is still recognisably itself, but spoiled. Difficult, and worth attempting.

Lowest tier: the substance is mis-categorised. It has crossed the line from what the body builds with to what the body discards. Not a matter of degree.

Sushruta does not argue this. He arranges the categories so that it follows, and moves on. It is the kind of compression that makes the samhitas rewarding and exhausting in equal measure.

This also explains why smell had to be the deciding register. Colour could not have carried this argument. Colour varies within a category — a thing can be many shades and still be itself. But the smell of a mala is a categorical signal: it says the substance now belongs to the other list. Prithvi, the element of embodiment and of what a thing substantially is, is exactly the right register for a claim about category membership.

One Instrument, Applied Twice

The chapter has two halves. Having graded the first substance, Sushruta turns to the second — artava — and here something structurally notable happens.

He does not build a second scheme. He re-applies the first one, unchanged, and says so explicitly: the states of artava vitiated by the doshas or by shonita “should be likewise considered”, and the measures described under the first head “should be likewise prescribed” for the second. The classical regimen of the rtumati in its own right is a separate subject, handled in our study of rajasvala paricharya in the Bhavaprakasha; what concerns us here is only the shape of the argument.

And the ladder repeats with the same rungs in the same order: those smelling cadaverous or of stale suppuration, those clotted, those thinned — and then, at the bottom, those emitting the smell of urine or of stool, which are placed beyond the reach of the measures available, the rest being amenable.

The same four smells. The same two at the bottom. The same silence from the colour scheme at the moment of verdict.

An instrument that is applied to one subject might be a convention of that subject. An instrument applied unchanged to a second, structurally different subject, with the same tiers falling out in the same order, is a general method. That is the strongest evidence in the chapter that Sushruta understood himself to be using a tool, not describing a peculiarity.

The symmetry is easy to miss in translation

English translations of the samhitas tend to expand the second half rather than mirror it, because “likewise” reads as an omission to a modern editor. In the Sanskrit the repetition is terse and deliberate: the point of saying likewise is that nothing new is needed. When a classical text repeats itself in compressed form, the compression is usually the claim.

Why the Chapter Is Called Shuddhi and Not Roga

The chapter's own title is Shukra-shonita-shuddhi Shariram. Not roga — not disorder. Shuddhi: soundness, unmixedness, the state of a thing being purely itself and nothing else.

That is a considered choice of word, and it fits everything above. A chapter about roga would be organised around what has gone wrong and what to do about it. A chapter about shuddhi is organised around how you establish whether a thing is what it claims to be. The first is pathology. The second is verification.

And verification is exactly what a smell test is. Smell is the classical world's fastest available assay: it requires no instrument, takes no time, cannot easily be argued with, and — crucially — it is very hard to fake. A substance can be made to look acceptable. Making it smell acceptable is a different order of difficulty. In a world without laboratories, the nose was the least corruptible witness available.

Three chapters, three questions

Sharira Sthana 1 asks: what is the body made of? Answer: the mahabhutas, arranged by the tattvas.

Sharira Sthana 2 asks: how do you tell whether a given material is sound? Answer: look, listen to what you are told — and then decide by smell.

Sharira Sthana 3 onward asks: what happens when sound materials combine? That is garbhavakranti, the descent of the embryo, and the beginning of classical embryology — a subject we have set out in our article on the shad garbhakara bhava, the six factors in Charaka's Garbhavakranti.

Chapter 2 is the verification step between the materials and their use. It is a chapter about quality control, sitting exactly where quality control belongs.

Gandha Pariksha Elsewhere in the Samhitas

Once you have noticed the instrument, it turns up repeatedly — and always at moments of consequence.

Sushruta, Sutrasthana 22. The chapter Vrana-srava-vijnaniya is devoted to reading the discharges of a wound, and it grades them by character and smell. For a surgical text this is not a minor passage: it is the primary means of judging how a wound is going, and Sushruta — whose reputation rests on the surgical portions — puts the nose at the centre of it.

Sushruta, Sutrasthana 23. Krityakritya sets out the general framework of sadhya and asadhya — what may be undertaken and what may not. Chapter 2 of the Sharira Sthana is a specific application of that general framework, which is why its three tiers feel so settled: the scaffolding was already built.

Charaka, Indriya Sthana. The entire section is given over to arishta — signs of grave prognosis — and it works through the sensory registers in turn, including the smells of the body. It opens, as noted above, with varna and svara: colour and voice.

The dravya side. The same instrument runs through the assessment of materials themselves. The nighantus — the classical materia medica lexicons, of which the Bhavaprakasha Nighantu is the best known — routinely record gandha as an identifying property of a substance alongside rasa, guna, virya and vipaka. Smell is how a classical practitioner confirmed that the material in front of him was the material he had asked for.

That last point is the one with a life outside the library, and it is where this article ends up.

How to Read a Chapter That Ranks by the Nose

Three things are worth carrying away from Sharira Sthana chapter 2, and none of them require any interest in the chapter's nominal subject.

One: a text's ranking reveals more than its descriptions. Sushruta describes on three registers and ranks on one. Anyone can list attributes; what a tradition decides by is where its real epistemology sits. When reading any classical chapter, find the sentence where a judgement is made and see which evidence survives into it.

Two: naming is a claim. Four of six categories are named by smell. That is a decision, made before any argument, about which property is most identifying — and it is visible in the vocabulary alone, without reading a word of the reasoning.

Three: the lowest tier is a category error, not a severity. The step from krichhra-sadhya to asadhya in this chapter is not the step from bad to worse. It is the step from a dhatu that is spoiled to a dhatu that now smells like a mala. The tradition's harshest verdict is reserved not for the most damaged thing but for the thing that has changed what it is.

Gandha as Evidence: Three Things and What They Smell Of

There is one entirely practical idea in all of this, and it survives the journey out of the eleventh-century manuscript intact.

What a thing smells of tells you what is in it. That is the whole of the classical instrument, stripped of its context. A material with a named aromatic ingredient in it will smell of that ingredient, and if it does not, either the ingredient is not there or it is not there in the quantity claimed. It is the cheapest verification anyone has ever devised, it needs no equipment, and it is available to you in about two seconds.

So, in the spirit of the chapter: here are three things from our shelf, described by what they smell of and why — the named material in each that accounts for its smell, read off the ingredient list. No other claim is being made about any of them.

Gulab Jal — smells of rose, because it is rose

Steam-distilled (bhaap aasvan) from fresh homegrown roses — Shatapatri in the classical vocabulary. There is one aromatic material in it and the smell is that material. If a rose water does not smell distinctly of rose, the nose has told you something the label did not.

The two-second version of Sushruta's method

Open it. Smell it. Then read the ingredient list and ask whether the two agree. If a product names an aromatic material prominently and does not smell of it, that disagreement is information — and it cost you nothing to collect. This is the only part of a classical text that you can use before breakfast.

Its Sanskrit title is Shukra-shonita-shuddhi Shariram — the chapter on the soundness of the two generative substances of the classical scheme. Two: mutra-gandhi, the smell of urine, and purisha-gandhi, the smell of stool. The interesting part is which smells are not in the lowest tier — kunapa (cadaverous) and puti-puya (of stale suppuration) are frankly worse to encounter, and they sit in the middle tier. The distinction appears to be categorical rather than one of degree. Mutra and purisha are the two principal malas, the substances the body has finished with, so a dhatu that smells of them is no longer presenting as a dhatu at all.
