Jalaukavacharana, the application of the leech, is the long second half of Ashtanga Hridaya, Sutrasthana 26. Having finished with the twenty-six sharp instruments, Vagbhata turns to the instruments that are not sharp: the leech (jalauka), the horn (shringa), the bottle gourd (alabu) and the earthen pot (ghati yantra). What follows is not a chapter about a disease. It is a chapter about an animal and three pieces of apparatus — how to tell a bad leech from a good one, what to feed it, how often to change its water, and how a gourd with a lamp inside it draws by itself.

Raktamokshana and jalaukavacharana are clinical procedures, performed where they are practised today by qualified, registered practitioners in a clinical setting with sterile single-use material. Jalaukavacharana: What Sutrasthana 26 Does After the Blades

The twenty-sixth chapter of the Sutrasthana of the Ashtanga Hridaya is called Shastra Vidhi, and for its first thirty-odd verses it behaves as that name promises: twenty-six sharp instruments, the iron they are forged from, the actions they perform, the faults that disqualify them, the wool-lined case they are kept in. We read that half in a separate article on Vagbhata's twenty-six sharp instruments.

Then, around verse 34, the chapter changes subject without changing title. For the next twenty verses Vagbhata is no longer describing metal. He is describing a worm, a horn, a vegetable and a pot, with the same flat, specifying attention he gave the blades. This is the half that deals with raktamokshana, and within it the long central passage on jalaukavacharana.

It is worth being clear about what kind of writing this is. Not folklore, and not a proof of anything: a technical specification, telling a reader how to recognise the right animal, keep it alive, judge whether it has done its work, and retire it. The editor of our edition summarises the chapter in a sentence at its close: it has discussed twenty-six sharp instruments, and accessory instruments such as shringa, alabu and jalauka.

That word, accessory, is the hinge. Vagbhata has already built a category for things that do an instrument's work without being instruments: in the first half of this chapter he calls them anu-shastra, and fills it with leaves, fingernails, rock crystal, alkalis and fire. The leech joins that list, and the classical author is not squeamish about it at all. The animal is equipment, and equipment has a specification.

Shad-Vidha Raktamokshana: Six Methods, and What Each One Reaches

Before the leech gets its own passage, Vagbhata sets out the whole field. Bloodletting is done by six methods, listed in this order:

  • Pracchana — scarification, a set of small incisions.
  • Siravedhana — venepuncture, opening a vessel.
  • Jalauka prayoga — the application of leeches.
  • Shringavacharana — the use of a horn to draw.
  • Alabu — the bottle gourd, used for cupping.
  • Ghati yantra — the same done with an earthenware pot.

What makes it more than a list is the principle attached to it, stated twice — here, and again at verses 53 and 54 at the very end of the chapter, as though it were the thing he most wanted remembered. The six methods are not interchangeable. Each is matched to a depth.

Method matched to depth (AH Su 26, vv. 53–54)

Pracchana is for blood gathered and fixed at one spot — ekadeshastham grathitam. Jalauka is for blood lying deepavagadhe. Shringa, alabu and ghati yantra are for blood lodged in the layers of the skin — tvaksthe. And siravyadha, the opening of a vessel, is for blood that is vyapaka: spread everywhere, circulating.

A second grid is then laid over the first, by dosha rather than depth: shringa, jalauka and alabu match vata, pitta and kapha respectively. The reasoning is given in terms of the apparatus itself. The horn is ushna and snigdha, hot and unctuous, so it suits vata. The leech is cold in nature, so it suits pitta. The gourd is sharp and hot in action, so it suits kapha.

The classification is not primarily about the patient. It is about the instrument: its temperature, its texture, how far it can reach. Vagbhata is doing to the leech and the gourd what he did to the twenty-six blades thirty verses earlier, giving each a property list and matching the list to a job. Jalayuka: An Etymology That Is Also an Address

The leech section opens with a half-line of word-play that the translation slightly flattens. The Sanskrit runs jalam asam ayur iti jalayukah: water is their ayus, their life, and so they are called jalayuka. The English edition renders the sense as the term applying to leeches because they are born in water and take their nutrition from water.

Ayus is the same word that stands at the head of the whole science: Ayurveda, the knowledge of ayus. Using it of a worm is not a flourish. Because water is the animal's life, every practical instruction in the next twenty verses turns out to be an instruction about water: which water it came from, how often it is changed, whether the animal moves freely in it afterwards. The etymology is the husbandry manual compressed.

Jalam asam ayur iti jalayukah: water is their life. Every husbandry rule in the chapter is a rule about water.

It also sets up the single criterion that runs through the whole passage. Vagbhata sorts leeches into two classes — savisha and nirvisha, with poison and without — and the sorting is done overwhelmingly by where the animal was found and what colour it is. Clean water gives one class. Foul water gives the other. For an author with no microscope, provenance is the available proxy for safety, and he uses it ruthlessly.

Savisha Jalauka: The Leeches the Text Rules Out

Verses 35 and 36 describe the leeches that must be discarded. The Sanskrit is unusually dense, a single compound doing most of the work: dushtambu-mrit-su-bheka-ahi-shava-kotha-mala-udbhavah, arising from foul water and mud, from the putrefaction of frog (bheka), snake (ahi) and corpse (shava), and from their mala, their waste.

Then the visual marks, which are what a person actually has at a pond's edge. The text calls out leeches that are rakta (red), shveta (white), intensely black (bhrisham krishnah) or algae-coloured; sthula and picchila, thick and slimy; carrying indrayudha markings, rainbow-coloured stripes running lengthwise up the back; and romasha, hairy.

The half-verse that closes the passage is the operative one: savisha varjayet. Those with poison, one should reject. Vagbhata then states what follows if the rejected kind is used anyway, and names the counter-measures. We do not reproduce that list here, for reasons set out in the section on what this article withholds.

Two things are worth noticing. Every criterion is available to the naked eye at collection (colour, thickness, surface, stripe, hair); nothing requires a test or a wait. And the provenance rule is not arbitrary: frog, snake and corpse are the three sources of decay a person at an Indian pond in the rainy season would actually meet. It does the job a modern sourcing standard does, excluding a whole population for where it has been rather than testing individuals.

Nirvisha Jalauka: Clean Water, Green Back, Yellow Belly

Verses 37 and 38 give the mirror image, and they are shorter, because a specification for the acceptable case can afford to be. Shuddhambujah punah — those born, again, of clean water. Then four marks: shaivala-shyava, the dark green of pond algae; vritta, round in section; nila-urdhva-rajayah, with dark blue lines running up the back; kashaya-prishthah, an orange-brown or astringent-coloured back and sides; and kinchit-pita-udarah, a slightly yellow belly.

Notice how ordinary this is as natural history. A round body in section, a dark dorsal stripe, a paler yellowish belly: these are field marks, written the way field marks are still written. The classical author is not describing a symbol. He is describing an animal the reader has to pick out of a pond, in a hurry, and be right about.

The two lists also differ instructively. The savisha description is longer, more anxious and organised around exclusions, the same shape Vagbhata used for the metal instruments earlier in the chapter, where eight shastra dosha get more attention than the virtues do. It appears to be how he thinks about equipment generally: define the failure modes precisely, and let the acceptable case be whatever is left.

The Twelve Names: What the Ashtanga Sangraha Adds

The Ashtanga Hridaya gives two classes and their marks. It does not name individual kinds of leech. The twelve names that are usually quoted in this connection come from somewhere else, and the edition is careful to say so: they are printed as editor's Notes, attributed to the thirty-fifth chapter of the Sutrasthana of the Ashtanga Sangraha, Vagbhata's own longer, older prose work.

So this is a case of the same author twice, at two levels of detail. The Hridaya, the condensed verse recension, keeps the diagnostic marks and drops the taxonomy; the Sangraha keeps both. Six names on each side — 6 + 6 = 12:

Savisha (with poison) — Ashtanga Sangraha, Sutrasthana 35

Krishna, the colour of black collyrium, with a broad head. Karbura, as long as an eel, with raised stripes across the abdomen. Alagarda, looking as if covered with hair, with large sides and a black mouth. Indrayudha, with rainbow-coloured stripes running lengthwise on the back. Samudrika, dark yellow, with variegated flower-like spots. Gochandana, with a tail forked like a bull's scrotum, and a small mouth.

Nirvisha (without poison) — Ashtanga Sangraha, Sutrasthana 35

Kapila, its two sides the colour of orpiment, its back smooth and green as a pea. Pingala, reddish-brown, round-bodied, quick-moving. Shankhamukhi, liver-coloured, with a long sharp mouth, and quick to take hold. Mushika, the shape and colour of a rat's tail, with a foul smell. Pundarika, green as a pea, with a mouth like a lotus. Savarika, slimy, green as a lotus leaf, eight inches long — and reserved, the note says, for veterinary use.

Every one of the twelve is named for a resemblance, drawn from a tight local vocabulary: a mineral pigment (kapila, orpiment), a garden vegetable (the green pea), a domestic animal's anatomy (gochandana), a flower (pundarika, the white lotus), a conch, a rat. This is the logic Vagbhata used for the sharp instruments earlier in the chapter, where the editor's phrase is namanugata rupa: the form follows the name, so the name itself is the drawing. Someone who has never seen a shankhamukhi can still find one, because the name contains the picture.

Note which item breaks the pattern. Savarika is listed among the harmless kinds but set aside for animals rather than people, and the reason given is simply its size. That is not a safety judgement but a dosing one, and it leads straight into the next rule.

Four to Six Angula: A Size Rule That Sorts Patients from Elephants

The edition gives a maximum length of eighteen inches for a leech, says that those of about four to six inches are the ones used on human beings, and assigns the larger animals to elephants, horses and the like.

Two cautions about those numbers. The unit in the Sanskrit is the angula, a finger-breadth; inches are the translator's conversion, not the text's measure. And the angula is itself a relative unit rather than a fixed one. The sound way to read the rule is proportional: an upper limit, a working band well below it, and everything above the band for large animals.

What is striking is that the rule exists at all. Veterinary practice, ashva-ayurveda and gaja-ayurveda, was a real and separate branch of the classical literature, and here it surfaces for a single line, purely to explain what the oversized animals are for. The author is not discarding them; he is reassigning them.

Sharad, Gloves and an Earthen Pot: Keeping a Living Instrument

This passage separates the leech from every other instrument in the chapter. A blade is kept in a wool-lined case; a leech has to be kept alive, and the text accordingly reads less like surgery and more like animal husbandry.

Collection is placed in sharad, the autumn: the season classical Ayurveda associates with blood, and the one Charaka assigns to the care of rakta in Sutrasthana 24. The collector covers the hands. The animals go into earthenware or a wide-mouthed vessel with water, and the water is changed every two or three days.

Then feeding, and the list is specific: lotus tubers and stalks, pond algae, dried animal matter. After seven days the colony moves to a fresh container. At verse 45 the reason for all this changing is finally stated, and it is an acute observation: leeches kept in a vessel can themselves become savisha by lying in their own saliva, urine and faeces. So the container is renewed, with fresh water and mud, every three or five days.

The rule underneath the rules. The two-to-three-day water change, the seven-day transfer, and the three-to-five-day container change all answer the same problem: the medium goes foul, and a leech kept in foul medium becomes, by the chapter's own definition, the kind of leech you must throw away. The classification at verse 35 was about provenance. The husbandry at verse 45 is the recognition that provenance keeps happening — that an acceptable animal can be turned into an unacceptable one by storage alone.

Verse 39 closes the section with three disqualifications, and they are the practical residue of everything above. A leech is not to be used when it has not fully disgorged what it took; when it has been applied too frequently; and when it is sluggish in water — when, having been returned to the water that is its ayus, it will not move properly in it.

Hamsa-Nyaya: The Simile at the Centre of the Chapter

At the point where the leech has taken hold, the text makes its one openly literary move. The leech, it says, draws off only the vitiated blood, as the hamsa takes only the milk from a mixture of milk and water.

The image is one of the most heavily worked similes in Sanskrit literature. The hamsa, the swan or bar-headed goose of the poets, is credited with ksheera-neera-viveka, the discernment that separates milk from water, and in philosophical writing it is the standard figure for an intelligence that takes the essence and leaves the rest. Bringing it into a technical chapter on apparatus credits the animal not merely with a function but with discrimination.

This is the text's claim, and we are reporting it rather than endorsing it. A leech draws what is in front of its mouth; the proposition that it selects vitiated blood and leaves sound blood is a classical explanation, not a demonstrated mechanism, and it is not offered here as one. What is genuinely interesting is the work the simile does inside the chapter's own logic. Every other method in the list of six is controlled from outside, with the operator deciding depth, placement and quantity. The leech is the one method where the text hands judgement to the instrument, and that transfer of control is why the very next verse has to supply an external check.

Reading a Leech by the Way It Swims

That external check is the quietly impressive part. Having said the animal knows what it is doing, the text at once gives two ways to verify it.

The first is at the site: when pricking and itching are felt where the leech is attached, the reading is that it has moved past the vitiated blood and begun to take sound blood, and it is removed. Sensation is the stop signal.

The second comes after removal, and is a small diagnostic system of its own. The leech is made to disgorge, with salt and oil at its mouth and the body stroked downward after dusting with fine rice flour, then returned to water and watched. Verse 44 reads the result three ways:

Complete. The animal moves freely in the water and its appetite returns. This is the wanted result.

Excessive. The animal becomes very weak, and may die.

Incomplete. It does not move, or moves sluggishly — and the procedure of disgorging is repeated.

Look at what this is. Behaviour in water is a read-out of an internal state that cannot be inspected directly, with three graded outcomes and a defined action for the failure case. The same criterion (does it move properly in water) also appears at verse 39 as a disqualification. The chapter has one test applied at two moments: once to decide whether an animal may be used at all, and once to decide whether it has been properly emptied after use.

Rakta-Mada and the Seven-Day Rest

Two short rules close the husbandry material, and both concern the animal's welfare rather than the patient's. The first is a rest interval: a leech that has been used is not used again for seven days. The second is what the edition renders as guarding against blood intoxication, rakta-mada: the state of an animal that has taken more than it can process and has not been properly emptied.

These are equipment-maintenance rules in form, but unmistakably about keeping a living thing alive. Read alongside the changing water, the feeding list and the three-way check on disgorging, the picture is of a colony maintained continuously, animals rotated through use and recovery: a reusable instrument with a duty cycle, and, unlike a blade, one that can be worn out by working too often. It is where this passage stops resembling anything in the first half of Shastra Vidhi. The shastra kosha protects steel from damp. Here, the thing protected is an appetite.

Shringavacharana: Seven Angula of Horn

With the leech finished, the chapter turns to the three suction methods. The working details of all three come from the editor's Notes, again citing Ashtanga Sangraha, Sutrasthana 35.

Shringavacharana is the use of a horn, of cow or buffalo, to draw. The specification is dimensional in the manner of the whole chapter: a horn of about seven inches, its circumference a thumb's width at the base and a black gram seed's width at the tip. The base is wrapped with clean cloth, and the draw is made by mouth at the narrow end.

The Hridaya's own contribution here is verse 50, and it is a restriction rather than a method. Blood vitiated by kapha is not to be drawn with shringa. The reason given is physical: the thickness of that blood. For vata and pitta, the horn is allowed.

That is a specification written in terms of viscosity: the instrument has a defined aperture, the aperture limits what passes through it, and the text refuses the instrument for material that will not pass. It is the reasoning behind the chapter's earlier insistence on properly smelted iron, a property of the material matched against a property of the job.

Alabu: The Gourd That Makes Its Own Vacuum

The alabu — generally known, the note says, as tumbi — is the bottle gourd, the ordinary Indian lauki, dried to a hard shell. The preparation is domestic: a medium-sized fruit, the stalk end cut away, the pulp removed, the shell kept.

Then the part that makes the passage worth the trip. A lamp or a piece of lighted cotton is put inside the shell, and the mouth is closed against the surface so tightly that no outside air enters. The flame goes out. A vacuum forms, and it is the vacuum that draws. After ten to fifteen minutes the gourd is lifted away.

This is fire-cupping, the same physics used by cupping practitioners today: a flame inside a sealed vessel, then a pressure difference across the rim. The edition attributes the effect to the flame exhausting the oxygen inside the fruit, and here an honest distinction has to be drawn. That explanation is Vidyanath's, written in the twentieth century in the vocabulary of modern chemistry. The classical text says the lamp is placed inside and that suction follows. Oxygen is the editor's gloss, not Vagbhata's word, and this article does not pretend otherwise. What the classical author had was the reliable, repeatable effect, and the sequence he specifies produces it.

The ghati yantra gets one sentence: the same method, with an earthenware pot in place of the gourd. One is the vessel a household already has in its kitchen garden, the other the vessel it has on the shelf. Neither has to be made by a smith, which is exactly why they sit in the accessory category rather than among the twenty-six.

Anala-Samyoga: Why the Gourd Is Refused When Pitta Is Involved

Verse 49 is the one place in this half of the chapter where Vagbhata gives an explicit causal reason for a restriction, and the reason is the apparatus, not the patient.

Alabu and ghatika are not to be employed where the blood is vitiated by pitta. The stated ground is tasam anala-samyogat: because of their conjunction with anala, with fire. They are, instead, for kapha and vayu.

The inference is compact. These two methods work by combustion; pitta is, in classical physiology, the principle of heat and transformation. An instrument whose operating principle is fire is refused where heat is already the problem. Whether or not one accepts the physiology, the form of the reasoning is worth noticing: the classification follows from the mechanism. Vagbhata has not observed that gourds disagree with pitta and recorded it. He has looked at how the gourd works and derived the restriction.

Set beside verse 50's refusal of the horn for thick kapha blood, a pattern appears. Each suction method is barred from exactly one dosha, and each bar is justified by a physical property of the device: the fire inside the gourd, the narrow tip of the horn. These are the two things any engineer asks about a pump: what drives it, and what is its throat.

Pracchana: Bound Above, and Four Things Never Cut

Verses 51 and 52 cover pracchana, scarification, and are almost entirely a list of cautions. The limb is first bound firmly and evenly above the site with a rope or strap. Then the exclusions, which the Sanskrit gives as a single compound of things to be avoided — snayu, sandhi, asthi and marma: tendons, joints, bones and the vital points. The incisions run upward from below, and are not to be too deep, too wide, or transverse.

The marma carry the most weight there. They are the classically enumerated vital points, mapped in the Sharira Sthana of the Sushruta Samhita and in the Bhavaprakasha's account of the joints, and avoiding them is close to a first principle of the classical surgical literature. Sushruta's own venesection chapter builds a scheme of twenty named faults around what happens when the rule is broken.

The specification of direction (upward, not sideways, not too deep) does anatomical work without anatomical vocabulary. It is a rule a practitioner can follow with no diagram, and that is the register of the whole chapter: a few things to avoid, stated in terms of what the operator can see and feel.

Paschat-Karma: Haridra, Madhu, Guda, and a Correction About Cold

Verse 46 deals with the site afterwards. Where blood remains at the point of application, the site is rubbed with haridra (turmeric), madhu (honey) and guda (jaggery), washed with cold water, and covered with a cloth pad soaked either in shatadhauta ghrita, ghee washed a hundred times, or in pastes of sheeta-virya dravyas — substances cold in potency. Three are named: yashtimadhu (liquorice), chandana (sandalwood) and ushira (vetiver).

Then verse 55, the last verse of the chapter, and it is a correction, which is an unusual way for a chapter to end. If cold applications are used on the region after bloodletting, vata becomes aggravated, and the text names what follows in terms of the site's own behaviour: pricking, itching and swelling. The instruction is to pour warm ghee over the part instead.

Two instructions about temperature, nine verses apart

Verse 46: wash the site with cold water; apply cold-potency pastes. Verse 55: cold applications on the region aggravate vata; use warm ghee. The edition does not reconcile them, and neither will we. The most defensible reading is that the two verses address different moments — the immediate dressing of the point, and the later state of the surrounding part — but the text as printed does not say so, and we are not going to supply a join that is not there.

The three named sheeta-virya dravyas recur constantly in classical cosmetic formulations quite apart from this context: chandana, ushira and yashtimadhu appear together in recipe after recipe across the Bhavaprakasha and the Chakradutta. Their presence here is the classical author reaching for the group he always reaches for when he wants something cooling on a surface.

Prastha Matra: The Numbers, and Which of Them Are the Editor's

The edition gives quantities for a single letting in three grades: one prastha at most, half a prastha, or a quarter. It then converts: 768 ml, 384 ml and 192 ml. Those figures are internally consistent (768 halves to 384 and quarters to 192) but they are a modern equivalence, and the value chosen for the prastha varies considerably between editors and regions. The classical measure is the prastha; the millilitres are Vidyanath's, and we flag them as his rather than pass them on as the text's own.

One more short rule accompanies them. If blood has been displaced and has stagnated deep in the wound, it turns sour overnight, and a second letting is indicated. The reasoning is mechanical rather than theoretical: something has moved, it has settled where it should not, and time makes it worse.

Where the Chapter Ends, and What Sutrasthana 27 Opens With

The colophon on the last page of our window is explicit about what has just finished. It names the work, the Ashtanga Hridaya Samhita, composed by Acharya Vagbhata, son of Sri Vaidyapati Simhagupta; the section, Sutrasthana; and the chapter, the twenty-sixth, called Shastra Vidhi.

That settles a small matter: the running header on several pages of this printing carries a different chapter name and number. The colophon is part of the text rather than the printing, and it reads shastravidhir nama shadvimsho'dhyayah — the twenty-sixth chapter, named Shastra Vidhi. Everything in this article belongs to chapter 26.

Manjistha and padma. Sutrasthana 27 opens by giving the colours of sound blood as plant names.

Immediately after the colophon, Sutrasthana 27 opens: Siravyadha Vidhi, venesection. Its first move is a simile drawn from irrigation. As water runs out and the plants die when the bank of a field is cut, so, the text says, the vitiated blood comes out when a vessel is opened. It then sets out the marks of shuddha rakta, sound blood, entirely in colour words: padma, the lotus; indragopa, the brilliant red rain-insect; and manjistha, the madder root, a dye plant whose colour was the standard reference for a particular red, discussed separately in our article on manjistha and the skin.

What This Article Withholds, and Why

Two passages here have been described but not reproduced, and it is better to say so than leave the gaps unexplained. The first is the bracketed passage after the hamsa simile, listing the conditions for which the text says leech application brings relief. The second is the list of effects it attributes to using a savisha leech.

We withheld both for the same reason. This is a commercial site that also sells Ayurvedic products, and an article pairing a named condition with a procedure, on a page that anywhere else mentions a product, starts to read as a therapeutic claim however carefully the sentence is built.   • Verse numbers quoted here (35–38 the two classes, 39 disqualification, 44 the readings after disgorging, 45 the container, 46 the site afterwards, 49 and 50 the two refusals, 51 and 52 pracchana, 53 and 54 method matched to depth, 55 the closing correction) were read off page images of this edition rendered at 110 dpi, not off the PDF's text layer, which is reliable for the English and unusable for the Devanagari.
  • The twelve leech names and the working details of shringa, alabu and ghati yantra are the editor's Notes, attributed in the edition to Ashtanga Sangraha, Sutrasthana 35. They are not verses of the Ashtanga Hridaya, and this article does not present them as such.

Jalaukavacharana means the application of the leech — jalauka, the leech, and avacharana, the act of employing it. It is the classical name for one of the six methods of raktamokshana, and it occupies the long central passage of Ashtanga Hridaya, Sutrasthana 26, from roughly verse 34 to verse 46. This article describes what that passage says: a historical and textual account, not instruction. The procedure itself is clinical, and performed only by qualified practitioners.

Which chapter of the Ashtanga Hridaya describes leeches? +

Sutrasthana 26, the chapter called Shastra Vidhi. Its first half covers the twenty-six sharp instruments; from about verse 34 to the colophon at verse 55 it covers bloodletting and the instruments that are not sharp — the leech, the horn, the bottle gourd and the earthen pot. The chapter number comes from the colophon, which reads shastravidhir nama shadvimsho'dhyayah, the twenty-sixth chapter named Shastra Vidhi.

Verses 53 and 54 match each to a depth: pracchana for blood fixed at one spot, jalauka for deep-lying blood, shringa, alabu and ghati for blood in the layers of the skin, and siravyadha for blood that is spread throughout.

How does the text tell a poisonous leech from a safe one? +

Almost entirely by where it came from and what it looks like. Verses 35 and 36 describe savisha (poisonous) leeches as arising from foul water and mud and from the decay of frog, snake and corpse: red, white, intensely black or algae-coloured, thick and slimy, marked with rainbow-coloured lengthwise stripes, and hairy. Verses 37 and 38 give the nirvisha kind as born of clean water, dark green like pond algae, round in section, with dark blue lines up the back and a slightly yellow belly.

What are the twelve types of leech named in Ayurveda? +

Six poisonous — Krishna, Karbura, Alagarda, Indrayudha, Samudrika and Gochandana. Six non-poisonous — Kapila, Pingala, Shankhamukhi, Mushika, Pundarika and Savarika. An important point of attribution: these names are not in the Ashtanga Hridaya itself. The edition prints them as editor's Notes, attributed to Ashtanga Sangraha, Sutrasthana 35 — Vagbhata's own longer prose work. Each name describes a resemblance, so that the name carries the picture.

What is the swan simile in the leech passage? +

The text says the leech draws off only vitiated blood, as the hamsa takes only the milk from a mixture of milk and water. The hamsa is the classical figure for discrimination — ksheera-neera-viveka, the separating of milk from water. It should be read as the text's own explanation rather than as a demonstrated mechanism. Notably, the verses immediately following supply external checks anyway: sensation at the site as a signal to remove the animal, and the way it afterwards moves in water as a check on whether it has been emptied.

How were leeches kept and fed, according to the chapter? +

Collected in sharad, the autumn, with the hands covered; kept in earthenware or a wide-mouthed vessel of water, the water changed every two or three days; fed on lotus tubers and stalks, pond algae and dried animal matter; moved to a fresh container after seven days. Verse 45 gives the reason for all the changing: leeches lying in their own waste can themselves turn savisha. A leech that has been used is rested for seven days.

How does the alabu (bottle gourd) work? +

A dried bottle gourd is cut at the stalk end and emptied of pulp. A lamp or lighted cotton is placed inside, and the mouth of the shell is closed against the surface so that no outside air enters. The flame goes out, a vacuum forms, and the vacuum draws. After ten to fifteen minutes the gourd is lifted away. This is fire-cupping, and the same physics is used in cupping today. The explanation in terms of the flame consuming oxygen is the modern translator's; the classical text describes the lamp and the resulting suction, not the chemistry.

Why is the gourd refused when pitta is involved? +

Verse 49 gives the reason as anala-samyoga — the conjunction with fire. Because alabu and ghati yantra work by combustion, and pitta is the principle of heat in classical physiology, an instrument driven by fire is refused where heat is already the issue; those two are assigned to kapha and vayu instead. Verse 50 makes the matching restriction for the horn: shringa is not used for blood vitiated by kapha, because of that blood's thickness.

Is this something a reader should ever attempt? +

No. Jalaukavacharana and raktamokshana are invasive clinical procedures, carried out where they are practised today by qualified, registered practitioners under clinical conditions with sterile single-use material.
