The short version
The fourth chapter of the Sharira Sthana of the Sushruta Samhita does something no other passage in early Indian medicine does quite so plainly: it puts a number on the skin. Not one number — a graded series, each layer given its own thickness as a fraction of a vrihi, a grain of rice. Underneath the skin it then names a second graded series, the kalas, the membranes that hold each dhatu in place. The chapter is not advice. It is a ruler.
What follows is a close reading of that ruler: the unit, the order, the arithmetic that quietly closes, and the reason an ancient surgeon needed the measurement in the first place.

Where Sharira Sthana 4 Sits in the Sushruta Samhita
The Sushruta Samhita divides itself into six sthanas, and the third of them — the Sharira Sthana — is the anatomy book. It is short, ten chapters only, and it moves in a deliberate order: the philosophical ground first, then the examination of the living body, then conception and the growth of the garbha, and then, in the fourth chapter, the thing that grows out of all of that. The chapter carries the name Garbha-Vyakarana Sharira — the analysis, or the unfolding, of the embryo. Vyakarana is the same word Sanskrit uses for grammar: a taking-apart into named components.
The chapter opens by listing what preserves the life of the foetus — pitta, shleshma, the bodily vayu, the three qualities of sattva, rajas and tamas, the five sense organs, and the self. That is Sharira Sthana 4.2 in the standard printed translation, and it reads as a continuation of the previous chapter rather than a new subject. Then, without transition, the text stops being philosophical and starts counting.
"Seven folds or layers of covering are formed and deposited," it says, and adds the image that the whole passage turns on: they form the way layers of cream form on the surface of boiling milk. That is Sharira Sthana 4.3, and everything this article is about begins there.
It matters that this is an embryology chapter and not a chikitsa chapter. Sushruta is not describing what to do about a surface. He is describing how a surface came to have depth at all — the layers arrive during formation, in sequence, the way cream arrives on milk that has been left on the fire. The measurement follows from that. If the layers were laid down one after another, they can be separated again, and if they can be separated they can be measured.

The two chapters immediately before this one set it up. Chapter 2 is Sushruta's examination by smell, an entire chapter built on one sense. Chapter 3 is the descent of the garbha and the month-by-month account of what forms when, including the famous exchange in which five teachers each name a different part as the first to appear and Dhanvantari answers that nothing is first. Chapter 4 is what you get when that formation is finished and someone asks how thick the result is.
Vrihi: The Grain That Measures the Twacha
Before any layer is named, the unit has to be understood, and the unit is strange enough that the translator stops to footnote it.
The Sanskrit for the first measurement is vriher ashtadasha-bhaga — eighteen parts of a vrihi. A vrihi is a grain of rice. But eighteen parts of a whole rice grain would be an absurd thickness for the outermost film of the skin, so the commentator Dalhana, whose Nibandha-samgraha is the standard gloss on the Sushruta Samhita, supplies the missing denominator: vrihi here stands for a measure equal to the twentieth division of a grain of rice. The unit is not the grain. The unit is a twentieth of the grain, and the numbers are counts of those twentieths.
This is worth pausing on, because it is the sort of detail that separates a text being read from a text being repeated. Without Dalhana the passage is unusable — the first layer would be eighteen rice grains thick and the whole scheme would collapse. With Dalhana it becomes a coherent scale in which the thinnest layer named is a quarter of a grain and the thickest is two whole grains.
Why a grain of rice, and not a finger?
Classical Indian measurement usually works from the body outward — the angula, the finger-breadth, is the standard unit for almost everything Sushruta measures elsewhere, including surgical instruments. Bodies differ, so an angula is a proportional unit; it scales with the person. A grain of rice does not scale. It is the same everywhere, it is in every household, and it can be split by eye into halves and quarters and, with care, twentieths. For something as fine as the film on the outside of a body, Sushruta reaches past the proportional unit for the absolute one.
There is a second reason a grain works here, and it is visual rather than metrological. Rice is what the layers are being compared to in size, and cream on boiled milk is what they are being compared to in behaviour. Both images come from the same kitchen. The chapter is doing something difficult — describing a structure nobody at the time could see — and it does it by reaching for the two most ordinary objects available.

The Sapta Twacha in Sushruta's Own Order
Here is the enumeration as Sharira Sthana 4.3 gives it, outward layer first, with the thickness as the printed translation states it and the twentieths made explicit using Dalhana's gloss. The Sanskrit names of the conditions Sushruta assigns to each depth are given as the text gives them, untranslated; the section after this one explains that choice.

Read the names down the left column and a pattern appears that the numbers alone do not show. The first two layers are named for appearance — Avabhasini reflects, Lohita is red. The third and fourth continue in colour: Shveta is white, Tamra is copper. The fifth changes register entirely: Vedini is from the root vid, to know or to feel, and it is the layer of sensation. The sixth, Rohini, is from ruh, to grow or to rise, the same root that gives aroha, ascent. The seventh is named for its function: it holds the mamsa.
So the naming itself descends. The outer layers are named by how they look, because from outside that is all there is. The middle layer is named by what it does to a sensation passing through. The inner layers are named by what they do structurally. The vocabulary moves from surface to function exactly as the enumeration moves from surface to depth, and there is no reason that has to be true — it is a choice, and a careful one.
A note on the spelling
Printed translations of the Sushruta Samhita vary in how they render these names, and older editions use diacritics that do not survive plain text well: Avabhasini appears as Avabhas'ini, Shveta as S'veta or Sveta, Rohini occasionally as Rohita by simple misreading. The seven names themselves are stable across editions. Where this article and a printed copy differ, the difference is transliteration, not content.
Fifty-Nine and Sixty: The Sum Hiding in the Layers
Set the thicknesses out in twentieths and read them as a sequence: 18, 16, 12, 8, 5, 20, 40.
The first five fall. Not evenly — the drops are two, then four, then four, then three — but they fall without exception, from eighteen twentieths at the surface to five twentieths at Vedini. Then the sequence reverses violently. Rohini is a whole grain, four times the layer above it. Mamsadhara is two grains, twice Rohini again. The thinnest layer in the body's covering is not the outermost one; it is the fifth, sitting at the hinge between a thinning stack and a thickening one.
Now add them. The five outer layers come to 18 + 16 + 12 + 8 + 5 = fifty-nine twentieths. The two deep layers come to 20 + 40 = sixty twentieths. The whole covering is 119 twentieths — a hair under six grains of rice.
Fifty-nine against sixty
The five thin layers and the two thick ones are within a single twentieth of a grain of being exactly equal. Whatever else the scheme is, it is balanced: half the covering is spread across five graded films, the other half is packed into two. A reader who wants to know whether a set of classical figures was transmitted intact or drifted in copying has one honest test available, and it is this — do the sub-totals close? Here they close, and they close on a near-symmetry that nothing in the text points out.
We should be careful about how much weight that carries. Sushruta does not state the total, does not state the two halves, and never draws attention to the balance. It may be an artefact of a scheme built for other reasons. But it is checkable, it survives in every edition that prints the fractions, and it is the kind of internal consistency that copying errors usually destroy. Compare it with the arithmetic in the Bhavaprakasha's enumeration of the joints, where the sub-totals also close at several levels — the classical anatomists were counting, and their counts hold.

The outer layers and the deep ones come to almost the same amount - the covering divides very nearly in half.
One more thing the sequence tells us. A scheme invented to be tidy would descend smoothly, or hold constant, or halve. This one does none of those. It falls irregularly and then jumps by a factor of four. Irregularity of that kind is weak evidence of observation — someone separating actual layers and finding that the deep ones are much thicker than the superficial ones, which they are.
Avabhasini: The Layer That Carries the Complexion
The first layer gets more attention in the text than the other six put together, and it gets a footnote of its own in the printed translation.
Avabhasini is built from ava plus bhas, to shine — so, the one that shines back. Sushruta says it is called that because it reflects all colours and is capable of being tinged with the hues of all five material principles of the body. Then comes the claim that made the name stick: the complexion of a person is due to this first layer, and as the colour of an opaque body is due to the rays reflected from its surface, this layer is rightly named the reflecting one.
That is a genuinely interesting sentence, because it contains a theory of colour. Sushruta is not saying the outermost layer is coloured. He is saying that colour, in an opaque object, is a property of what its surface returns — and that the body's outermost film is therefore where its visible colour lives, no matter what lies underneath. The five mahabhutas may tint it, but the seeing happens at the boundary.
It is also, quietly, an argument for why the enumeration has to start where it does. If the visible surface is a distinct layer eighteen twentieths of a grain thick, then everything anyone can see of a body is that thin, and everything else is inference. The chapter spends the rest of its length on what cannot be seen.

Under one light, the same surface returns what it returns. Sushruta's argument for Avabhasini is an argument about surfaces.
A Depth Is Not a Diagnosis: What the Names Are Doing There
Every one of the seven layers is given, in the same breath as its thickness, a short list of conditions that classical nosology places at that depth. This is the part of the chapter a modern reader is most likely to misread, so it is worth being exact about what it is and what it is not.
It is a coordinate system. Sushruta is writing for a surgeon, and a surgeon needs to know how far in something sits before deciding anything at all. The names in the fifth column of the table above are not being explained, defined, described, or acted on — several of them are given as bare nouns with an "etcetera" attached, and the reader is assumed to know them already from elsewhere in the corpus. They are depth markers. They tell you that this named thing lives at eight twentieths and that named thing lives at forty, and that is the entire work they do in this chapter.
You can see this most clearly where the scheme is untidy. Kushtha appears twice, at Tamra and again at Vedini, in adjacent rows. A list of indications would not do that. A depth chart would, and does, because a category as wide as Kushtha — Sushruta's Nidana Sthana divides it into eighteen types — is not one thing at one depth. Similarly Vyanga, which Sushruta locates in the second layer here, gets an entire chapter of its own in the Chikitsa Sthana. The chapter in front of us has nothing to say about either beyond where they sit.
Scope of this article
This is a reading of a classical text, not diagnostic guidance and not a self-assessment tool. The Sanskrit condition names above are reproduced as scholarly vocabulary in the order the source gives them; they are deliberately left untranslated, because the classical categories do not map cleanly onto modern diagnoses and a confident-looking English gloss would invent a correspondence the text does not support. Nothing here is a description of what a reader should do, look for, or conclude about themselves or anyone else. The refusal to translate is a real editorial choice and it costs something — an English reader would find the passage more vivid with glosses attached. But the honest position is that a term like Kilasa or Charmadala carries a classical description that a modern label would flatten, and the chapter itself declines to define them. Where a reader wants those definitions, they belong to the nidana literature, which is where Sushruta's sixteen Nidana Sthana chapters put them, and not here.
The Thumb and the Vrihi-Mukha: Who the Ruler Was For
The skin passage does not end on the seventh layer. It ends on a caveat and then on a limit, and the two together say plainly what the measurement was made for.
The caveat first: these dimensions, Sushruta says, hold for the skin of the fleshy parts of the body, and not for the skin of the forehead or about the tips of the fingers. He knows the covering is not uniform. The seven-layer scheme with its fractions is a description of typical skin, not of all skin, and the exceptions are named rather than ignored.
Then the limit. Sharira Sthana 4.3 closes by invoking a surgical dictum — that an incision as deep as the thickness of the thumb may be made into the region of the abdomen with a vrihi-mukha instrument. The vrihi-mukha is one of Sushruta's named instruments; its name means rice-grain-mouth, after the shape of its point, and the same grain that measures the layers turns up in the name of the tool that cuts through them.
That is the whole argument for why this chapter counts. A practitioner about to open a body needs to know two things: how far in the structures lie, and how far in it is permissible to go. Sharira Sthana 4.3 supplies the first as a graded table and closes with the second as a hard ceiling. The enumeration is not decorative and it is not symbolic. It is the reference table for a knife.

The measurement exists for the cut. Sharira Sthana 4.3 closes on a limit, not on a description.
This is consistent with everything else we know about the Sushruta Samhita, which is the surgical text of the classical corpus — the one with the instrument catalogue and the operative procedures. The Charaka Samhita, whose interests are internal medicine and pharmacology, has no comparable passage; its own anatomy is organised around other questions, as the structure of its eight sthanas shows. Measuring the covering in twentieths of a rice grain is something you do when you are going to cut through it.
Sapta Kala: More Boundaries, Deeper In
Having finished with the covering, the chapter immediately starts a second enumeration, and it is built on exactly the same idea.
"The kalas too number seven in all," says Sharira Sthana 4.4, "and are situated at the extreme borders of the different fundamental principles of the organism." A kala is a membrane, a boundary layer — the thing that separates one dhatu from what surrounds it and holds it in its place. Where the twacha are the boundaries you meet going in from outside, the kalas are the boundaries you meet between one tissue and the next, all the way down.
Then the chapter gives its best image, in the Memorable Verses at 4.5-6. As the duramen — the dense heartwood at the core of a stem — is exposed to view by cutting into the wood, so the root principles of the body may be seen by removing the successive layers of its flesh. And the kalas themselves, the verse adds, are extensively supplied with snayu, bathed in mucus, and encased in a membranous covering.

The chapter's own simile: as the core of a stem is exposed by cutting into it, so the dhatus are reached by removing what lies over them.
The heartwood simile is doing precise work. It says that the body's interior is not a jumble but a series of concentric zones, that the zones have real boundaries, and that the way to find them is to remove what lies outside them in order. That is a description of dissection, offered as an analogy to something any carpenter had seen. It also sets up the relationship between the two enumerations: seven boundaries outside, seven boundaries inside, and the same method — go in one layer at a time and name what you cross.
Mamsadhara to Shukradhara, in Sushruta's Order
The seven kalas are given one per section, each with a Memorable Verse attached, from Sharira Sthana 4.7 through 4.20. They are named for what they hold.

The list runs outward-in and then downward, and the sequence tracks the dhatus themselves. Read against the classical account of the seven dhatus, five of the seven kalas are named directly after a dhatu or its container — mamsa, rakta, meda, and shukra are dhatus; the remaining three hold shleshma, purisha and pitta, which are not dhatus at all but the doshas and malas. So the scheme is not a mechanical copy of the dhatu list. It is a list of things that need holding, whatever category they belong to.
The fourth is worth a second look. Shleshmadhara is the membrane of the joints, and its Memorable Verse at 4.15 is the wheel-and-axle: as a wheel turns easily on a well-greased axle, so joints moistened by the shleshma these sacs contain admit of easy movement. That is a synovial membrane described functionally, in the eleventh or twelfth century of its transmission, by someone who had watched a cart.
The seventh is the one the tradition kept arguing about. Sushruta says shukra is coextensive with the whole organism, and his Memorable Verse at 4.21 gives two comparisons for it: like sarpi in milk, and like sugar in the expressed juice of sugarcane. Present everywhere, visible nowhere, extractable by a process. Centuries later the Bhavaprakasha takes up the same problem and the same simile, and we have a separate reading of that passage, including Vriddha Vagbhata's objection that a seventh kala with no address is not much of a kala.
Yatha Sarpi Payasi: The Similes Sushruta Argues With
Count the comparisons in this one chapter and something becomes obvious about how it was meant to be taught. There are five, and every one of them is drawn from a kitchen, a workshop or a pond.
- Cream on boiling milk (4.3) — how the layers of the covering are laid down, one over another, during formation.
- Heartwood exposed by cutting (4.5-6) — how the interior is reached, by removing what is outside it in order.
- Lotus roots and stems in ooze and water (4.8) — how the sira ramify through the flesh, growing and spreading together.
- Latex from a pricked plant (4.10) — why blood appears the instant the flesh is opened.
- A wheel on a greased axle (4.15) — why a joint moves without catching.
- Ghee hidden in milk, sugar hidden in cane juice (4.21) — how shukra can be everywhere and nowhere.
None of these is ornamental. Each one carries the precise structural point of the section it sits in, and each one is checkable by anyone who has boiled milk, split a log, pulled up a lotus, snapped a stem, greased an axle or made ghee. The chapter is describing structures that no student could be shown directly, and it does it by attaching each structure to something the student had already handled.
The phrase for the last of them is yatha payasi sarpih — as ghee in milk. It is one of the most durable images in Indian medical writing, and it recurs across centuries and across texts precisely because it solves an argumentative problem rather than a decorative one: how do you assert the presence of something that cannot be pointed at? You point at another thing that also cannot be pointed at, and that everyone agrees is nonetheless there.
Fourteen Boundaries: The Body Read as a Graded Depth
Put the two enumerations together and the shape of the chapter's whole claim appears. Seven layers of covering, measured in twentieths of a grain. Seven membranes below them, each at the border of what it holds. Fourteen named boundaries between the outside air and the deepest tissue, in a fixed order, and a method for reaching each one.
This is not how modern anatomy divides a body, and the correspondences people reach for — Avabhasini as the stratum corneum, Mamsadhara as fascia, Shleshmadhara as synovium — are approximate at best and misleading at worst. The translator of the standard English edition supplies several of them in parentheses and they are useful as orientation, but the classical scheme is not a rough draft of the modern one. It answers a different question. Modern histology asks what a tissue is made of. Sharira Sthana 4 asks how far in it is.
The chapter's real subject
Depth as a coordinate. Everything in Sharira Sthana 4 — the fractions, the order, the exceptions for the forehead and fingertips, the ceiling on the incision, the heartwood simile, the second series of membranes — is in service of one question: given a structure, how far below the surface does it lie? The chapter never advises anyone about anything. It locates.
The rest of the chapter, from 4.24 onward, turns to the formation of the organs and then to sleep, and it ends somewhere quite unexpected: with the heart named as the special seat of consciousness, folding like a lotus bud at its apex during sleep and expanding on waking, and with a long passage on nidra, day sleep and its opposite. That material is a separate subject and deserves its own reading; it is not covered here.
What This Article Does Not Reproduce, and Why
Sharira Sthana 4 is an anatomy chapter, so it carries less operational material than most, but it carries some, and the following is deliberately left out rather than summarised. This article does not reproduce:
- The permissible incision depth as an actionable figure or the instrument to which it is attached, beyond noting that the chapter closes on such a limit and that the limit is what the measurement exists to serve.
- Any correspondence between a named classical condition and a modern diagnosis. The fifth column of the first table is Sushruta's vocabulary, left in Sanskrit for the reason given above.
- Any method by which a reader might attempt to identify a depth, a layer or a condition on themselves or anyone else. There are no dose figures, no preparations and no procedures anywhere on this page, because the chapter's subject is structure and none of it is addressed to a patient.
- The chapter's later material on the formation of the organs, on nidra and on tandra, jrimbha and klama, which belongs to a different reading.
Two of these are editorial judgement and two are not negotiable. Classical surgical instruction is written for a trained physician working under supervision within a living tradition, and reproducing its operative detail on a page anyone can land on serves nobody. Saying so once, in a list, is more honest than scattering apologetic asides through fifteen sections.

Sharira Sthana 4 is a chapter of classical measurement written for a reader whose job was nothing like ours, and nothing on our shelf has anything to do with it. What are the sapta twacha, the seven layers of skin in Ayurveda? +
In Sushruta's order, outward first: Avabhasini, Lohita, Shveta, Tamra, Vedini, Rohini and Mamsadhara. The first four are named for how they look, the fifth for sensation, the sixth for growth and the seventh for what it holds. Each is given a thickness and a set of classical conditions located at that depth.
How thick is each layer, and what is a vrihi? +
A vrihi is a grain of rice, but the working unit is a twentieth of one, which the commentator Dalhana supplies. On that scale the thicknesses run 18, 16, 12, 8 and 5 twentieths for the outer five, then a whole grain for Rohini and two grains for Mamsadhara. The covering totals 119 twentieths, a little under six grains.
Why do the outer layers get thinner and then the deep ones get thick? +
The text does not say. What can be observed is that the five superficial layers fall irregularly rather than by any tidy rule, and that Rohini is four times the layer above it. One consequence worth noting is that the five outer layers add to fifty-nine twentieths and the two deep ones to sixty, so the covering divides almost exactly in half between many thin films and few thick ones.

Mamsadhara, Raktadhara, Medadhara, Shleshmadhara, Purishadhara, Pittadhara and Shukradhara, given in that order at Sharira Sthana 4.7 to 4.20. A kala is a boundary membrane sitting at the extreme border of what it holds. Five are named after a dhatu or its container; the other three hold shleshma, purisha and pitta.

As depth markers. The chapter is written for a surgeon who needs to know how far below the surface a structure lies, so each classical name is used as a coordinate rather than a subject. The names are not defined, described or acted on anywhere in the chapter, and several are given as bare nouns with an "etcetera" attached. Kushtha appears at two adjacent depths, which a list of indications would never do.
Is the sapta twacha the same as the layers modern anatomy describes? +
No, and the parenthetical glosses in printed translations should be read as orientation rather than equivalence. The classical scheme answers a different question. Modern histology asks what a tissue is made of; Sharira Sthana 4 asks how far in it lies, and organises everything around that.
Do the measurements apply to skin everywhere on the body? +
Sushruta says explicitly that they do not. The figures hold for the fleshy parts and not for the forehead or about the tips of the fingers. Naming the exceptions rather than ignoring them is one of the reasons the passage reads as observation rather than scheme.
